Initiatives: Diabetes For Life

The Diabetes for Life (DFL) project has been working to transform health system practices to enhance diabetes management and reduce health disparities in Memphis by:

Establishing a Quality Improvement Collaborative for diabetes care. Partnership agreements have been signed with six practice sites, paving the way for the establishment of a countywide quality improvement collaborative. Each has site identified an area of health system change that represents their ‘change goal’. Sites will be implementing these changes utilizing the Plan Do Study Act framework. The Healthy Memphis Common Table hopes to expand the collaborative by adding as many as 10 new practices.

Implementing patient health status logs. This practice change has allowed case managers to distribute logs to diabetes patients, provide training on their use and encourage them to discuss the logs with their providers. Clinic protocols were revised and logs are now part of the patient enrollment packet.
